Geriatric Care Managers are trained health-care professionals qualified to assess the biological, psychological, social and functional needs of older adults. They formulate care plans and recommend appropriate resources and support to older adults and their families. They function as an advocate, liaison, and/or support system for older adults with their primary health care providers, financial and legal team, as well as with long distance family members. Geriatric Care managers provide ongoing screening, monitoring and followup are provided to closely address any changes in the older adult’s daily life.

A complete Biopsychoosocial and Functional Assessment followed by an individualized care plan.
Coordination of Medical Appointments and Services
Alhzeimer’s and Dementia Care
Facilitation of communication between physicians, patients, and families
Provide skilled clinicians to attend medical appointments and translate the medical jargon to the client and/or family
Assist with medication management and wellness programs
Identify the need for home modifications and facilitate completion
Ongoing Monthly, Weekly, or Daily Monitoring based upon your preference
Review financial, legal and medical issues in order to offer referrals to geriatric specialists and conserve assets.
Bill Paying and Medical Claims Filing Services
Emergency Crisis Management, intervention and support.
Accompany clients home from the hospital and meet with discharge planners
Senior housing placements to retirement, assisted living, skilled nursing and rehab facilities.
Screen coordinate, and monitor in home help and other services

In Home Services Provided by Happily EverAfter Senior Living are billed at $150/hour, but are billed in 15 min increments. For example, a 30 min session with our providers is only billed at $75.

1 weekly call and 1-60 minute weekly supervisory visit ($750 value)
Weekly review of medications and assistance in setting up
Coordination with long distance family members
Ongoing communication with your Primary Care Physician
